Summary:

• Location: Brownsville, TX
• Work Mode: Field/Remote
• Duration: 3 months to start, potential to extend or convert to permanent
• Shift: Monday-Friday, 8 AM to 5 PM

• Conduct assessments of Level 2 Nonwaiver members.
• Perform field visits and manage related paperwork and tasks.
• Assist members in meeting unmet needs and provide education on medical benefits.
• Work schedule includes 3 days of member home visits and 2 days for documentation, tasks, emails, training, and audit corrections.
• Complete all documentation/Auth requests related to home visits within a 3-day TAT.
• Maintain audit scores of 95% and above.
• Ensure all tasks, emails, and trainings are completed by the due date.

• Education/Certification: LVN/SW required, LVN preferred.
• Licensure: LVN/SW required, LVN preferred.
• 4 years of experience in nursing, MCO, and home health.
• Proficiency in computer knowledge, especially MS Office.
• Consistent job history without frequent job changes.

• Experience working remotely.
• Self-starter and self-motivated individual.
• Bi-lingual skills preferred or ability to work well with a translator.
• Top 3 must-have hard skills:
  • Computer knowledge, especially Excel.
  • Experience with MCO.
  • Experience in Home Health.
