Job Description
Licensed Social Worker

Required Skills:
- Valid Louisiana State License as a Licensed Social Worker
- Strong knowledge of social work principles and practices
- Excellent communication and interpersonal skills
- Ability to assess patient needs and develop appropriate care plans
- Proficiency in documentation and electronic health records
- Experience working in healthcare or home health settings
- Bilingual abilities, particularly in Spanish
- Familiarity with community resources and referral processes
- Bachelor's or Master's degree in Social Work (BSW or MSW)
- Minimum of 1 year of relevant clinical experience
- Prior experience in a healthcare or home health environment is advantageous
- Ability to work 8-hour day shifts
- Willingness to travel locally if required
- Certification requirements must be confirmed with VHS upon application
- Valid driver's license and reliable transportation preferred
